With sausage McMuffin, it is very clear they used a smaller muffin on the top for the ad.

One time, a long time ago, I went into McDonald's and ordered something, I forget. Anyway, I said, "I want it to look exactly like the one on TV."
They laughed.

I always roll my eyes when people complain about how the sandwich doesn't look like it does in the ad. Even if I made something following a chef's exact instructions I know it wouldn't look like theirs.
